Forms and Paperwork

This page will have all of the documents that you will need to have signed as well as maps of the region, fund raising letters you may wish to send out, and brouchures to share with your friends to convince more people to help!

Forms everyone must fill out -
Note: if you are a Bonaventure Student or under 18, you also must have parents/legal guardians complete the forms. So plan your time accordingly!

Any one who is not taking the bus or who is not a student must also fill out this form and send back.

These MUST BE FILLED OUT OR YOU WILL NOT BE ALLOWED ON THE TRIP. NO EXCEPTIONS!

Brochure/Pamphlet-- Warning, this is a large MS publisher File and does not open in all versions of the software.

Flyers--January 24, 2006 (have important dates etc) Hang them around campus, in stores, etc.

Sponsorship form--Updated January 25, 2006
